At Santa Rosa Orthopaedics You May Schedule An Appointment Without Being Referred By Another Doctor. However, If Your Insurance Plan Requires A Referral From Your Primary Care Provider, You Will Need To Obtain This Referral Before Arriving For Your Appointment.

Our Offices Are Open From 8:00 Am Until 5:30 Pm Monday Through Friday. Patients Can Schedule Appointments From 8:30- 5:00. Early Notification Of Cancellations Is Appreciated So That Your Appointment May Be Filled With Another Patient And Avoid Delays. You May Be Charged For A Missed Appointment, Untimely Cancellation Of An Appointment, Surgical Cancellation Fee For Late Cancellation And Or Failure To Comply With Surgical Instruction/S.

Please Bring All Pertinent Medical Records With You From Your Primary Care Provider Or Other Orthopaedic Physician. These May Include:

We Look Forward To Meeting You At Your Scheduled Appointment. Please Arrive At Least 30 Minutes Before Your Scheduled Appointment Time. Allow At Least One And A Half To Two (1.5 To 2) Hours For Your First Appointment.


At The Time Of Your Appointment Please Bring The Following:

  • The Two Completed And Printed Forms Standard Patient Demographic Form & Standard Patient History Form
  • All Current Medical Insurance Cards
  • Referral Form, If Required By Your Health Plan
  • Recent X-Rays Or MRI Studies
  • A List Of All Current Medications

Santa Rosa Orthopaedics Participates In Most Major Insurance Plans And Will Bill All Insurance Plans, Whether Participating, (Contracted), Or Not Contracted.

Please Bring Your Insurance Card At Each Visit To Our Medical Or Physical Therapy Offices.

In An Effort To Keep Medical Costs Down We Require All Managed Care Co-Payments Be Made Prior To Seeing Your Physician Or Therapist.  It Is The Responsibility Of The Patient To Obtain The Necessary Referral Prior To Seeing A Physician Or Therapist For The First Time Or For A New Injury Or Complaint.

For Outside Referrals We Can Also See Workman’s Comp Patients, But The Authorization, Which Has To Be Generated By The Treating Physician’s Office, Needs To Indicate That We Do Not Go Through Third Party Billing And The Carrier Must Accept Billing Directly From SRO. Work Comp Authorization Must State: Therapy To Be COMPLETED At Santa Rosa Orthopaedics.

We Try Our Best To Handle Your Requests For Prescription Refills Quickly. To Order A Refill, Please Call Your Pharmacy, And They Will Request Your Refill From Our Office.

Please Do Not Contact The Physician On Call After Normal Business Hours. Our Physicians Handle A High Volume Of Emergency Calls And We Ask That Our Patients Play An Active Role In Monitoring The Status Of Their Own Prescriptions, And Plan Ahead For Weekends And Holidays. In General We Would Appreciate It If You Would Allow At Least 48 Hours For Prescription Refills To Be Processed As In Many Cases Your Physician May Be In Surgery And Not Receive Your Request Until The Following Day.
